HEALTHY START SUPERVISOR
Phoenixville Site

This position offers a unique opportunity to manage the daily activities of a community based office for the Healthy Start Program in rapidly growing Chester County, Pennsylvania. Healthy Start is one of the services offered by MCHC.

Key Responsibilities:

  • Supervise (including hiring, training, scheduling and performance management) a team of Family Health Advocates who provide outreach, home visiting, support, and referrals for pregnant women and families.   This dedicated group of paraprofessionals are community residents who provide culturally sensitive links between pregnant and parenting women and the complex health care and social services system.
  • Oversee entire caseload of the Site including: assigning new cases; assessing risk levels; assisting in the development of individualized client service plans; and monitoring compliance with agency policies and program protocols.
  • Provide direct services to clients, especially those with complex problems or requiring crisis intervention.
  • Establish and maintain linkages with community health and social service resources (working to identify and reduce access barriers to health and human services); advocate directly with provider agency staff to obtain needed services and resolve complaints; and represent the Healthy Start Program at interagency meetings.
  • Maintain a client case management data base and prepare regular reports on Program services and activities.

Qualifications:

The ideal candidate will have a Master’s degree in nursing, social work or a relevant human services field and a minimum of two years of proven ability to effectively supervise staff in the human services field. Candidates with a Bachelor’s degree and five years supervisory experience will be considered.  Excellent communication skills (verbal and written) in English and Spanish required as well as experience using Microsoft Office. The individual must be able to work flexible hours (some evenings and weekends) and legally drive their own vehicle on a daily basis.

SPANISH-BILINGUAL CLERICAL ASSISTANT

One full time (37.5 hours/week) position is available requiring travel to several Healthy Start offices and the Family Center. This position requires bilingual (Spanish/English) communication skills. Benefits for this position include subsidized health insurance and 401(k) plan with employer match.

Responsibilities:
Enters data from selected Healthy Start and Family Center client data forms into client database.
Prepares data reports.
Performs other work, such as answering the telephone, contacting clients, photocopying, mailings, and other tasks as required.

Qualifications:
High School diploma or G.E.D.
Bilingual (Spanish/English) communication skills.
Strong data entry skills alpha and numeric.
Knowledge of Microsoft Office (Excel, Access, Word).
Able to work one evening per week.
Valid driver’s license, a reliable car, and proof of car insurance.
One year experience doing data entry and general office work.

Hours/Salary/Benefits:
37.5 hours per week
Salary: $11.50 per hour
