BILINGUAL FAMILY BENEFITS ADVOCATE

The Family Benefits Advocate supports Maternal and Child Health Consortium of Chester County’s (MCHC) Family Benefits Program, a county-wide effort to enroll uninsured pregnant women, parents and their children in Medicaid, CHIP and SNAP (Food Stamps) and other public benefits, as needed.

 

Key Responsibilities Include:

  • Implementing an annual outreach plan to raise community awareness about Medicaid, CHIP and SNAP as well as educating families about SNAP and health insurance benefits.
  • Providing enrollment and recertification assistance at out-stationed sites throughout Chester County.
  • Serving as liaison between clients and the County Assistance Office and providing follow up assistance, as needed, to ensure that applications include all required supporting documents and are processed in a timely manner.
  • Maintaining and tracking relevant documentation to support the application process as well as supporting electronic submission of applications and maintenance of relevant databases.
  • Keeping current regarding information on health care services and food programs in the county and making referrals, as needed.

Qualifications:

  • Must be fully bilingual in Spanish and English with excellent oral and written communication and interpersonal skills. 
  • Associates’ degree with minimum of one year of experience providing social services. Additional education and/or work experience may be substituted for these requirements.
  • Exceptional attention to detail with strong organizational, analytical, critical thinking and problem-solving skills. Must be able to effectively coordinate multiple activities simultaneously and maintain and track a vast amount of information and paperwork while meeting tight deadlines.
  • Capacity to work independently as well as collaboratively in a fast paced, high-performing team environment.
  • Demonstrated capability to successfully interact and work collaboratively with individuals from a wide variety of backgrounds and organizations, and feel comfortable working in non-MCHC settings.
  • Able to work flexible hours (evenings and weekends) and travel overnight.
  • Proficiency in Microsoft Office.
  • Ability to successfully complete criminal and child abuse background checks.
  • Valid PA Drivers’ license, car insurance, daily access to a car and cell phone required

HEALTHY START MANAGER - COATESVILLE

In Chester County, Pennsylvania as throughout the United States, African Americans have twice the rate of pre-term and low birth weight babies than the general population.  Maternal and Child Health Consortium of Chester County (MCHC), through its Healthy Start Program, provides services to at-risk families to help eliminate this disparity. 

This position offers a unique opportunity to manage the daily activities of a community based office for the Healthy Start Program.  MCHC is seeking a leader who has the capability and experience to drive the organization’s outreach, health education, and social service efforts in Coatesville, Pennsylvania particularly to the African American community.

Key Responsibilities Include:

  • Supervise a team of Family Health Advocates who provide outreach, home visiting, support, and referrals for pregnant women and families. This dedicated group of employees are community residents who provide culturally sensitive links between pregnant and parenting women and the complex health care and social services system.
  • Oversee entire caseload of the Site including: assigning new cases; assessing risk levels; assisting in the development of individualized client service plans; and monitoring compliance with agency policies and program protocols.
  • Provide direct services to clients, especially those with complex problems or requiring crisis intervention.
  • Establish and maintain linkages with community health and social service resources (working to identify and reduce access barriers to health and human services); advocate directly with provider agency staff to obtain needed services and resolve complaints; represent the Healthy Start Program at interagency meetings.
  • Ensure all staff follow agency and program policies and monitor progress of team toward achieving the Healthy Start program objectives. Maintain a client case management data base and prepare regular reports on Program services and activities.

Qualifications:
The ideal candidate will have a Master’s degree in Public Health, Social Work or a relevant human services field and a minimum of two years of proven ability to effectively supervise staff in the human services field. Candidates with a Bachelor’s degree and five years supervisory experience will be considered.  Experience with community-based programs, particularly in African American and Latino communities, is highly desirable.

The individual must be able to work flexible hours (some evenings and weekends) and possess a valid driver’s license, car insurance and daily access to reliable vehicle. Must have ability to successfully complete criminal and child abuse history background checks.

HEALTHY START PROGRAM DIRECTOR

Maternal and Child Health Consortium of Chester County (MCHC), the leading advocacy and service agency for women, children and families in Chester County, is seeking a Director with the experience and leadership ability to manage our largest initiative the Healthy Start Program. This Program annually provides prenatal, home visiting, and case management to over 1000 parenting and pregnant women in the County.

We are seeking an experienced leader to continue the growth and development of the Healthy Start Program.

Key Responsibilities Include:
The Healthy Start Program Director provides leadership, training and coaching to Managers at community sites located throughout Chester County in order to: develop outreach plans which publicize programs and meet enrollment objectives; achieve program goals and objectives; develop and ensure adherence to program policies and protocols; effectively manage program staff including recruiting, hiring, training, interpreting and applying employee policies and assessing staff needs; and proactively resolve programming, staffing, and administration issues.  The position also oversees related services including prenatal and parenting classes, depression screening and support groups, and car seat safety events.

This “hands on” role assesses the quality of program operations, achievement of objectives, barriers to achievement of objectives and solutions to overcome barriers. In addition, the Director is responsible for the accuracy and timeliness of program data collection; the preparation of timely reports on program activities, utilization, and quality assurance; and, working with other members of the senior management team, determines annual program objectives and provides budget oversight.  This individual is also expected to be a leader in the maternal and child health community and apply the latest research to the Program’s design and delivery.

This senior position represents MCHC in interfacing with a wide range of audiences including community partners, funders, and relevant state and national associations.

Requirements/Desired Competencies Include:
MCHC is seeking a visionary leader who is interested in joining our values driven organization– a creative, energetic individual who will lead by example. The ideal candidate must possess:

  • A track record of at least 7+ years at a senior management level achieving successful results in program planning and implementation, staff development and management, budgeting, and evaluation preferably in the maternal and child health or related field. Demonstrated capacity for managing complex programmatic challenges.
  • A passionate commitment to improving the health of women and children.
  • Strong work ethic with the proven ability to develop and lead a team.
  • Master’s degree in social work, public health or related human services field and relevant professional licensure.  
  • Strong oral, written communication, interpersonal and negotiation skills. Bilingual ability in Spanish and English a plus but not required.
  • Skilled at adjusting to changing priorities and managing workload changes and impacts, including multi-tasking to meet deadlines and changing staffing requirements.
  • Ability to collaborate with the IT team on the implementation of technology solutions to enhance services
