A History of Building Healthy Communities

Since 1991, Maternal and Child Health Consortium (MCHC) has forged pathways to resources and opened up educational opportunities for families and young children in the Chester County region.

Our team brings high quality evidence-based education and trauma-informed care to community members across diverse backgrounds. We strive to advance equitable access to ensure that families and children have what they need to start healthy, stay healthy, and succeed in school.

Family Support & Education Programs

We are a team dedicated to building healthy communities and opportunities to grow for children and parents.

Services are voluntary, provided at no-cost to participants, and available in English and Spanish.

Healthy Start

Families build the foundation for a lifetime of good health with personalized home-visiting support during pregnancy and early parenthood (children ages 0-18 months).

Family Benefits

Community members access essential medical care and nutritious food with guidance through the public benefits enrollment process.

The Family Center

Children grow their love for learning and skills to succeed in kindergarten and beyond through personalized home-visiting support (children ages 0-5 years).

Pregnant and parenting women and infants received a continuum of care from a qualified Community Health Worker in 2023.

Eligible individuals (80% children) were enrolled into health insurance and food benefits assistance programs in 2023 to access important health care and nutritious food.

Kids achieved learning milestones and successfully graduated the Summer Kindergarten Transition Program in 2023.
